Asthma review (children)

Use this service to submit a routine review of a child’s asthma.

You can use this service if:

• you are registered at the surgery
• you have been invited to do so
• the patient is aged 4 to 11

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• the details of the child you are completing the form on behalf of
• information about the child’s asthma
• a peak flow reading
• the child’s height and weight
• any history of exposure to second hand smoke
