To register with the surgery:
- via the NHS App
- online via NHS: register with a GP surgery: Alliance Teaching Practices, Coventry
The practice has a requirement to assign an ‘accountable GP’ to all patients on the practice list. All patients will be assigned to a GP in the practice and this will be recorded on their medical record as their ‘usual GP’.
You do not need proof of address or immigration status, ID or an NHS number. You will be given a registration form and patient questionnaire by a receptionist to complete.
A receptionist will then call you to schedule an appointment with a healthcare assistant to book you a new patient health check
